If you need an appointment, schedule it today. We do not need your old records before our first visit at the new clinic. Our staff will make your first appointment comfortable. Bring your insurance card and identification to your visits.

Once we have your signed authorization, we will work diligently to have your records transferred.

If you are a new patient, welcome! If you are re-establishing with Lucinda, welcome back!

This is an excellent time to update your contact information. Our new system is much more comprehensive and will aid in providing exceptional care! You can wait until the day of your appointment if you prefer. In that event, please arrive 30 minutes early.

Medical Records
your records are not automatically transferred to the new location. The law requires your signature on the release form. If you fax it ahead, we can have your records in advance of your appointment.

Can I pick them up myself?
The law will allow you to retrieve a copy of your records in person. However the custodian clinic, currently Dr. Kristina Harp at Lake Oswego Medical Clinic, has up to 30 days to prepare them and may charge you a fee. We can do it for you and simplify the transition.

How do I refill my prescription?
Until you are established with Lucinda at her new location, your doctor is officially Doctor Kristina Harp. Dr. Harp will complete refills until you can be re-established. Call to make an appointment, print off the forms to the left and we will take care of your records and get you back into care with Lucinda.
